My site was opening and automatically redirecting to some other sites is my site has been hacked? How to identify whether my site has been hacked or not. Help me.

Here are few signs that indicate hacked or not.
1. Your website gets tarnished
2. Drastic changes in website performance
3. You can find broken code in your source page
4. Unknown plugins added automatically
5. Your site has been blacklisted
6. Distrustful activities in website and server logs
